Clinic Management Software Malaysia: 2026 Price Guide

clinic management software malaysia

Clinic management software Malaysia clinics buy is priced in one of four ways: a monthly subscription per doctor, a monthly subscription per clinic, a per-user fee, or a one-off licence with a yearly maintenance charge. For a small GP clinic, the figure on the price list is rarely the full cost. Add 8% service tax on IT services, setup and training, data migration, WhatsApp or SMS credits, and an e-invoice module to meet LHDN rules. This guide explains each cost in ringgit, compares cloud with on-premise over three years, and shows how to get a fair quote. To see a cloud option built for Malaysian clinics, start with EMR software in Malaysia.

How much does clinic management software cost in Malaysia?

There is no published, independent price survey for clinic management software in Malaysia. Prices depend on the pricing model, the number of doctors and users, the modules you switch on, and whether the system runs in the cloud or on a server in your clinic. [NOT CONFIRMED: no non-vendor source publishes RM price ranges for clinic software in Malaysia. Ask three providers for written quotes and compare them with the tables below.]

Three things drive the price of clinic management software in Malaysia:

  • Pricing model. Per doctor pricing rises with every clinician you add. Per clinic pricing stays flat as you grow. A one-off licence moves the cost to year one and to your own IT budget.
  • EMR, appointments and billing are the base. Inventory, pharmacy, lab, insurance claims, teleconsultation and multi-branch control cost more.
  • Compliance work. The PDPA amendments, the LHDN e-invoice rollout and the service tax expansion all add cost. Some sits in the software price. Some lands on your clinic.

Clinic finances are tight. A Malaysian Medical Association study of nearly 2,000 clinics found that over 70% of GPs are struggling to stay open, with 21.1% earning under RM20,000 a month (The Vibes, 5 May 2026). A June 2025 report put a typical clinic’s operating cost at at least RM40,000 a month and listed “digital systems” among the rising costs (New Straits Times, 7 June 2025). Software should be a small share of that. Treat any quote that is not as a warning sign.

Pricing models: per doctor, per clinic, per user, one-off licence

Each model suits a different clinic. Price ranges are left out on purpose, because no non-vendor source publishes them for Malaysia.

Pricing modelWho it suitsWhat drives the costTypical range in RM
Per doctor per monthSolo GP, small specialist clinicNumber of doctors, plan tier, billing period (yearly plans are often discounted)Varies, ask for quotes
Per clinic per monthClinics with several part-time doctors or locumsNumber of branches, modules, patient volume capsVaries, ask for quotes
Per user per monthClinics with many doctors and few support staffEvery login counts, including receptionists and nursesVaries, ask for quotes
One-off licence plus yearly maintenanceClinics that want to own the system and run their own serverLicence in year one, then an annual maintenance contract, plus hardware and ITVaries, ask for quotes
Free or freemiumSolo clinics testing digital recordsLimits on patients, users or storage, paid add-ons for WhatsApp, e-invoice or backupsRM0 base, add-ons vary

Per doctor is the most common model for cloud clinic management software in Malaysia. Front desk users are usually free. It gets expensive when you add locums who see a handful of patients a week, so ask for a locum login at a lower rate. Per clinic pricing is one amount with a cap on patients, users or branches, so check what the next tier costs. Per user pricing charges every login and punishes clinics with a large front desk. A one-off licence is the traditional on-premise model. The licence is only part of the cost, and it leaves you alone with the security duties the PDPA amendments now place on data controllers.

Monthly cost of EMR software for a small clinic in Malaysia

For a small clinic with one or two doctors, the monthly cost of EMR software in Malaysia is the subscription plus service tax plus recurring extras. Build the real monthly number before you compare providers.

Cost lineWhat it isHow to find the number
SubscriptionPer doctor, per clinic or per user feeThe provider’s price list or quote
Service tax8% on IT services, which the RMCD guide says includes cloud services and software maintenanceAsk whether the quote is inclusive. Providers below RM500,000 in taxable services do not charge it
WhatsApp and SMS creditsAppointment reminders, lab results, follow-upsAsk for the per-message rate and a monthly estimate for your patient volume
E-invoiceIssuing validated e-invoices through LHDN’s MyInvois systemAsk whether it is included, an add-on, or missing
Backups and storageCloud storage limits, document and image uploadsAsk what is included and the overage rate
SupportPhone and chat support, training refreshersAsk what is included. Some providers charge for onsite visits

PwC’s 2025/2026 Malaysian Tax Booklet lists IT and digital services at the general 8% service tax rate, with a RM500,000 registration threshold. A Royal Malaysian Customs Department guide of 26 February 2026, summarised by Wolters Kluwer (30 July 2026), places cloud services, software provision and software maintenance within taxable IT services under Group G of the Service Tax Regulations 2018. A quote that says “plus SST” is 8% higher than it looks.

Your own fees are separate. Since 1 July 2025, private healthcare attracts 6% service tax, but only on non-citizens and only for facilities above RM1.5 million turnover, per the Ministry of Finance statement reported by Malay Mail (9 June 2025) and PwC’s TaXavvy Issue 16 (10 June 2025). Malaysians are exempt. If your clinic is above the threshold, the billing module must split citizen and non-citizen invoices.

A quick example: a clinic with two doctors, one receptionist and a nurse pays two seats under per doctor pricing and four under per user pricing. At 40 patients a day, one reminder each is about 1,000 WhatsApp messages a month, so the per-message rate matters.

Cloud vs on-premise clinic management software Malaysia cost

Cloud clinic management software in Malaysia costs more in subscriptions and less in everything else. On-premise costs less in subscriptions and more in hardware, IT time, backups and compliance. Over three years, the items that are not on the price list decide it. Fill in your own quotes, because no non-vendor source publishes RM figures for these lines.

Why PDPA tilts the comparison

The Personal Data Protection (Amendment) Act 2024 came into force in three stages, 1 January, 1 April and 1 June 2025, per the gazette dates reported by Christopher & Lee Ong (27 December 2024). From 1 April 2025, data processors such as cloud providers must meet the Security Principle directly. From 1 June 2025, breach notification and data protection officer appointment became mandatory, as LAW Partnership explains (13 May 2025).

The Commissioner’s guidelines, summarised by DLA Piper (March 2025), require you to notify the Commissioner within 72 hours of a breach likely to cause significant harm, notify affected patients within seven days of that, and keep a breach register for two years. A compromise of health data counts as significant harm. A data protection officer is required above 20,000 data subjects, or 10,000 for sensitive data, so a chain may need one. Rödl & Partner (23 January 2025) notes the maximum fine rose from RM300,000 to RM1 million, with up to three years’ imprisonment.

On-premise clinics carry that risk alone. Cloud clinics share the security duty with a processor who is now directly liable for it. That is a real saving, as long as you confirm where the data is stored. Our guides to patient data privacy laws in India and data privacy laws in Kenya show how other markets handle the same question. On-premise still fits a clinic with poor internet, an in-house IT person and a strong reason to keep data in the building. Even then, budget for an offsite backup, because MyInvois needs a live connection.

Hidden costs that do not appear on the price list

Every clinic software quote has a second page. Ask for it.

LHDN’s revised e-invoice timeline, as reported by Malay Mail and Bernama (5 and 6 June 2025), gives three dates for clinics: 1 July 2025 for turnover above RM5 million to RM25 million, 1 January 2026 for above RM1 million to RM5 million, and 1 July 2026 for RM500,000 to RM1 million. Clinics below RM500,000 are exempt. Each phase has a six-month relaxation period for consolidated e-invoices, per Grant Thornton’s tax alert (11 June 2025). Confirm the current phase on the LHDN e-invoice page, because the timeline has moved twice.

Malaysian rules that change what your software must do

Price follows requirements. These rules shape what clinic management software Malaysia clinics run has to do, and so what it costs.

Act 586 and your medical records

Private medical clinics are registered under the Private Healthcare Facilities and Services Act 1998, administered by the Ministry of Health’s Medical Practice Division, with new applications on the myMedPCs portal. Section 16 requires records to be retained for inspection, and section 117 makes failing to take care of medical records an offence, with a maximum fine of RM10,000 for an individual and RM30,000 for a company, according to a 2023 statutory analysis of the Act. [NOT CONFIRMED: the minimum retention period in years for private clinic records under the Private Healthcare Facilities and Services (Private Medical Clinics or Private Dental Clinics) Regulations 2006. Check the Regulations and confirm with CKAPS.] The Malaysian Medical Council’s guideline on medical records (002/2006) adds that records are confidential and patients may request copies, so your software needs audit logs, role-based access and reliable backups. Our guide to opening a clinic in Malaysia covers the registration steps.

Is clinic software a medical device?

Usually not. The Medical Device Act 2012 covers software only when the manufacturer intends it for a medical purpose such as diagnosis, monitoring or treatment, according to the Medical Device Authority’s definition of a medical device (MDA/GD/0006). Scheduling, billing and record-keeping are not on that list. An AI diagnosis module might be. [NOT CONFIRMED: whether the MDA has published software as a medical device guidance that names administrative clinic systems as excluded. Ask the provider whether any AI or decision support feature has been assessed under Act 737.]

The national digital health plan

The Health White Paper, passed by the Dewan Rakyat on 15 June 2023, puts electronic medical records and a lifetime health record at the centre of a reform that reaches private providers in later stages. By the end of 2025, 412 government health clinics ran on the Cloud-based Clinic Management System, with all 1,137 due by the end of 2027, and the Ministry is engaging private operators on interoperability standards (The Vibes, 27 February 2026). The Health Minister has set a target of one citizen, one record by 2029, with public and private clinics interoperable (Malay Mail, 11 December 2025). So ask whether the system can export records in a standard format. One that cannot will cost you a second migration.

Revised GP fees

Since 2 April 2026, the GP consultation fee under the Seventh Schedule is RM10 to RM80, per P.U.(A) 150/2026, with specialist first visits at RM80 to RM235. Your billing module must hold the new schedule and apply after-hours and home-visit surcharges. Ask whether fee schedule updates are included.

Grants that lower the price

The Geran Digital PMKS Madani pays up to 50% or a maximum of RM5,000 of the invoice for up to three digital services from an approved partner, including e-invoicing, ERP and accounting, cybersecurity and digital payments. The business must be at least 60% Malaysian-owned, registered with SSM or a local authority, six months old, and have annual sales of at least RM50,000 (BSN). In May 2026 the Ministry of Digital confirmed it had cleared a RM19 million backlog of grant payments (Business Today, 12 May 2026). [NOT CONFIRMED: whether the grant is accepting new applications in the current cycle and the 2026 allocation. Check the BSN page and ask the provider whether it is an approved partner.] Budget 2026 also set aside close to RM1 billion in financing and grants through development financial institutions for digitalisation (The Star, 10 October 2025). Grants use the SME Corp definition: in services, micro means sales under RM300,000 or fewer than five staff, and small means up to RM3 million or under 30 staff. Almost every private clinic qualifies.

How to get a fair quote: step by step

  1. Count your seats. List every doctor, locum, nurse and receptionist who will log in. This decides the cheapest pricing model.
  2. List your must-have modules. EMR, appointments, billing and e-invoice are the base. Add inventory, pharmacy, lab, claims, teleconsultation or multi-branch only if you use them now.
  3. Set your compliance checklist. PDPA audit logs and access control, a known data storage location, MyInvois integration, the Seventh Schedule fee table, and 6% service tax billing if you are above RM1.5 million.
  4. Ask three providers for a written quote on the same seat count and module list. Ask each to state whether the price includes 8% service tax.
  5. Ask for the second page. Setup, training, migration, message credits, integrations, storage overage, custom forms and exit cost.
  6. Build a three-year total for each quote, cloud and on-premise. Include your own IT, backup and hardware costs for on-premise.
  7. Check the grant. Ask whether the provider is an approved Geran Digital PMKS Madani partner and what paperwork you need.
  8. Run a trial with real workflows. Register a patient, book, consult, bill, issue an e-invoice, send a reminder and export the record.
  9. Read the data clauses. Where is data stored, who owns it, how fast can you export it, and who notifies whom on a breach?
  10. Negotiate the term. Yearly billing is often cheaper. Ask for a price lock and a cap on renewal increases.

Common mistakes when buying clinic software

  • Comparing the subscription alone. Two quotes RM50 apart can be RM500 apart once setup, credits and tax are in.
  • Forgetting service tax. The 8% gap between “inclusive” and “plus SST” changes the ranking.
  • Paying per user with a large front desk. Count receptionists and nurses before you pick the model.
  • Buying on-premise without an IT budget. The licence is cheap. The server, backups, patches and breach readiness are not.
  • Ignoring e-invoicing. Clinics with turnover of RM500,000 to RM1 million have been in scope since 1 July 2026. Adding a module later costs more.
  • Skipping the data export test. If you cannot export records during the trial, you cannot when you leave.
  • Treating the grant as guaranteed. Apply before you sign and budget for the full price in case the grant is closed or delayed.
  • Locking in a multi-year term before a trial. A short first term with a price lock protects you.

Clinic management software Malaysia: common questions

How much does clinic management software cost in Malaysia?

It depends on the pricing model, the number of doctors or users, the modules and whether it is cloud or on-premise. No independent Malaysian price survey exists, so ask at least three providers for written quotes on the same seat count and module list, and add 8% service tax where the provider is registered.

Is there service tax on clinic software subscriptions in Malaysia?

Yes, where the provider is registered. IT services, including cloud services and software maintenance, are taxable at 8% once the provider passes RM500,000 in taxable services in 12 months. Ask whether your quote is inclusive.

Does my clinic have to charge service tax on consultations?

Only if your turnover is above RM1.5 million, and only on non-Malaysian citizens, at 6%, since 1 July 2025. Malaysians are exempt. Most small clinics are below the threshold.

When does my clinic need e-invoicing?

From 1 January 2026 if your annual turnover is above RM1 million to RM5 million, and from 1 July 2026 if it is RM500,000 to RM1 million. Clinics below RM500,000 are exempt. Confirm the current phase on the LHDN e-invoice page.

Is cloud or on-premise clinic software cheaper in Malaysia?

Cloud costs more in subscriptions and less in hardware, IT, backups and compliance. Over three years, the cost of running a server and PDPA breach readiness usually decides it, and cloud shares those duties with a provider who is directly liable under PDPA.

What does the PDPA amendment mean for my software budget?

Since 1 June 2025 you must notify the Commissioner within 72 hours of a serious breach and affected patients within seven days, keep a breach register for two years, and appoint a data protection officer if you pass the thresholds. The maximum fine is RM1 million. Your software needs audit logs, access control and backups that support those duties.

Can I get a government grant for clinic software?

The Geran Digital PMKS Madani pays 50% of the invoice up to RM5,000 for approved digital services from an MDEC-listed partner, if your business is at least 60% Malaysian-owned, SSM-registered, six months old and has sales of at least RM50,000. Check the BSN page for the current application status.

Is clinic management software a medical device in Malaysia?

Not in most cases. The Medical Device Act 2012 covers software only when it is intended for a medical purpose such as diagnosis, monitoring or treatment. Scheduling, billing and records do not meet that test, but an AI diagnosis module might. Ask the provider.

Which pricing model is cheapest for a solo GP?

Usually per doctor per month, because one seat covers the whole clinic and front desk users are often free. Per user pricing costs more once you add receptionists and a nurse.

How long must a private clinic keep medical records in Malaysia?

Act 586 requires records to be retained for inspection and makes failing to care for records an offence. [NOT CONFIRMED: the exact retention period in years under the 2006 Regulations. Confirm with CKAPS before setting a deletion policy.]
